Registration for Elementary Faith Formation

• Completed forms can be emailed to abell@youngstowndiocese.org, dropped off at the Rectory, or mailed to

• Children in Kindergarten through 8th grade are invited and welcomed to join our Elementary Faith Formation.

• The program is for children NOT attending a Catholic elementary school.

• Reminder: children and youth who are preparing for First Reconciliation, First Communion or Confirmation must participate in Faith Formation classes.

• Please note that the Family Fee does not need to be included in when you register.
